so Long to "Fee For Service". it's time for a change in mindset.

Value-based care requires a change in mindset from one patient at a time (fee for service), to stepping back at a macro level and looking at an entire population. Then determining how to deliver better care at a lower cost and then acting on it. Our work with practices is aligned with both the quadruple aim and the medical home concepts. Our vision for medical practices is fully aligned and supports value-based care thinking.

Our Vision

1. Improve Population Health

A program that enables practices to generate greater revenues, for the same effort, while helping practices become more efficient and productive with their time. The ultimate goal is recognition from a national quality organization. Our synergistic process involves education, meticulous planning, documentation and process improvement.

2. IMPROVE THE EXPERIENCE OF CARE

Achieving improved patient experience of care (including quality and satisfaction) is accomplished by identifying new and innovative focused patient care strategies. These improvements benefit staff and clinicians satisfaction.

3. REDUCE COSTS FOR THE POPULATION

Achieving a reduction in the per capita cost of health care by improving related quality measure performance. Key Initiatives:  aligning the practice’s quality measures across different payers, responding to payer provided data with a focus on tangible improvements in clinical outcomes and coordinating patient care across care settings.

4. Improve Staff and clinician satisfaction

Improving the work life of clinicians and staff. A vital initiative as clinician burnout continues to rise. Key factors to increasing satisfaction include improvement in order, organization, communication, cohesion, promoting core values, and prioritizing/delegating tasks appropriately.
